Who can apply to the Hybridge Smile Foundation?
A.
When working with Hybridge Smile Foundation candidates within your practice, elect a patient who presents an impactful struggle with their oral health, who may tug at your heartstrings, but cannot otherwise financially afford treatment on their own. This patient will present as the ideal candidate and case that you will want to donate your time to as well.

When will the candidate hear from the HSF?
A.
With the high demand of those needing the services of the HSF, it can takeup to 8 weeks before you may be notified if you are a candidate.
Q.
What forms do I have the potential candidate fill out?
A.
The candidate will first be required to fill out the Initial Application and Financial Aid Form to be reviewed by the participating practice (forms provided on the HSF website). If the patient is selected, please follow up to have the patient sign the Hybridge Warranty Form and the Hybridge XD Expectations Form ("XD Info Sheet"), which can be downloaded at HybridgeEDU.com.

How many cases can I submit each year?
A.
At this time, the Hybridge doctor is able to submit for one to two arches depending on criteria.
Q.
How will I know if I am eligible to participate with the Hybridge Smile Foundation?
A.
The Hybridge certified doctor will be notified via email by the Hybridge Smile Foundation of their practice's eligibility, which will be determined by the number of arches facilitated with the Hybridge Laboratory in the previous calendar year. 15-19 arches = 1 HSF arch, and 20+ arches = 2 HSF arches.
Q.
Can I charge my patient or offer a discount?
A.
The HSF asks the doctor to donate their time and proceed with the case pro bono. On double arch cases, you may, however, proceed with one arch pro bono, charging for the second arch, if it is instead your preference to give away two arches to two different patients this year.
Q.
Will I receive free dental implants?
A.
Please note that the HSF does not participate in the process of requesting implant donation. It is the responsibility of the participating clinician to contact their implant representative if they would like to request donation of the implants (in these cases, a Sunshine Act form will most likely be provided by your representative for you to sign).
Q.
How will the Hybridge Dental Lab know not to charge me for the HSF case Iam submitting?
A.
For your approved HSF patient submissions, please notate the letters "HSF" on the top right of the lab slip and highlight them, so that the Hybridge Laboratory Customer Service Team is alerted to your HSF pro bono case submission.
