Practice to submit all forms to the Hybridge Smile Foundation.

HSF Smile Design Team committee to review applicant.

Once applicant is approved, HSF alerts the practice on which monththe case can be submitted to the Hybridge laboratory.

At the patient's initial records appointment, please capture a patient interview via video (phone video is acceptable) on their dental journey, and what led them to now.

Case begins in laboratory.

Practice videotapes the final delivery to capture the final words of the patient.

HSF may use photos and video footage for website and or any marketing needs. Please submit to HSF all before and after photos and videos.
