This Privacy Policy explains how Hybridge, LLC and its affiliated dental practices in Buffalo, NY (Buffalo Dental Smile Design P.C.), Rochester, NY (Elmwood Dental Group – Implant and Restoration, P.C.), and Syracuse, NY (Syracuse Dental Smile Design P.C.), (“Hybridge,” “we,” “us,” or “our”) collect, use, and share information through hybridgeimplants.com (this “Website”), our forms, our email and text communications, and our digital advertising (collectively, the “Services”). By using the Services, you agree to this Privacy Policy.

Independent Providers and Health Information
Each Hybridge dental implant office is independently owned and operated by a licensed dental practitioner who has completed Hybridge Full Arch Dental Implant Protocol certifications. Hybridge, LLC does not provide dental care and is not responsible for the care delivered by any provider. We encourage you to meet your local Hybridge provider before deciding to proceed with any treatment.

This Privacy Policy does not cover protected health information (“PHI”) that you provide to a Hybridge provider in connection with treatment. That information is governed by the provider’s obligations under the Health Insurance Portability and Accountability Act (“HIPAA”) and applicable state law, including the privacy practices of your local provider. Please do not send confidential health information through this Website or by email, as electronic communications are not guaranteed to be secure.

Information We Collect
We collect information in the following ways:

Information you provide. When you submit a contact or appointment form, register for a seminar or event, or otherwise communicate with us, we collect information such as your name, mailing address, phone number, and email address, along with anything else you choose to share. Information you submit is typically shared with your local Hybridge provider so they can follow up with you.

Photographs you upload. If you use our Smile Generator, you upload a photograph of your face, which is transmitted to and processed by SmileGen, operated by BiteBot, our third-party smile simulation provider, to produce an AI-generated simulated image. If you submit your contact information through the tool, we receive a notification that includes your photograph and the generated image, and we share them with your local Hybridge provider. We describe this in more detail in our Consumer Health Data Privacy Notice. Uploading a photograph is optional.

Information collected automatically. Like most websites, we and our service providers use cookies, pixels, and similar technologies to collect your IP address, browser and device information, the pages you visit, and how you interact with the Website. We also use session-recording and heatmap technology (such as Lucky Orange) that captures your interactions with the Website, including mouse movements, clicks, and scrolling, to help us understand how visitors use our site and improve the experience. This helps us understand usage, improve the Services, and support our advertising. For details, see our Cookie Policy. We also use a content delivery network and website security service (Cloudflare) that processes your IP address and request information to protect the Website against automated abuse and to deliver content efficiently.

How We Share Information
We share information with your local Hybridge provider so they can respond to your request. We also share information with service providers who help us operate the Website and run our business, and with advertising and analytics partners. When we share information with a service provider, we require that it keep the information confidential and use it only to perform services for us.

  • Website hosting and technical service providers
  • Smile simulation and AI imaging providers, and their hosting and storage subprocessors

  • Communications and messaging service providers
  • Professional advisors, such as auditors and attorneys
  • Content delivery network and website security providers
  • Legal, regulatory, and law enforcement authorities, where required

We use third-party advertising and analytics technologies, such as Google Analytics and the Meta Pixel, that may transmit online identifiers and browsing activity to those companies for analytics and cross-context behavioral advertising. Under some state privacy laws, these activities may be considered “sharing” or “selling” of personal information, and you have the right to opt out (see “Your Choices and Rights” below).

We may also disclose information when required by law, in response to a subpoena or similar legal process, to protect our rights and safety or those of others, and in connection with a merger, acquisition, or sale of assets.

Cookies and advertising.
You can manage cookies through the “Manage Cookie Preferences” control on our site and through your browser settings. We honor Global Privacy Control (GPC) browser signals as a valid opt-out of the sale or sharing of personal information.

Analytics opt-out. You can opt out of Google Analytics using Google’s browser add-on at tools.google.com/dlpage/gaoptout.

Email and text. You can unsubscribe from marketing emails using the link in any message, and stop marketing texts by replying STOP.

State privacy rights. Residents of certain states, including California, have rights to access, correct, or delete their personal information, and to opt out of its sale or sharing. Text Messaging and Mobile Opt-In
When you provide your phone number and opt in on one of our forms, you consent to receive text messages from Hybridge or your local Hybridge provider, such as appointment-related messages and information about our services. Message and data rates may apply, and message frequency varies. You can opt out at any time by replying STOP, and reply HELP for assistance.

Your mobile opt-in information and consent to receive text messages will not be shared, sold, rented, or otherwise transferred to any third party or affiliate for their marketing or promotional purposes. We share mobile information only with service providers that help us operate our messaging program, such as our messaging platform and telecommunications providers.

Retention
We retain personal information for as long as necessary to fulfill the purposes described in this Privacy Policy, unless a longer retention period is required or permitted by law, such as to meet legal, accounting, or reporting obligations. When determining how long to keep information, we consider its amount, nature, and sensitivity, the purposes for which we collected it, and applicable legal requirements. When information is no longer needed, we take reasonable steps to delete or de-identify it. Photographs uploaded to the Smile Generator and the simulated images generated from them are deleted by our simulation provider within 30 days. Where a photograph and generated image are sent to us with your inquiry, we retain them only as long as needed to respond to you and discuss your treatment options, and we delete them when they are no longer needed for that purpose. If you become a patient, they become part of your dental record and are retained under that provider’s HIPAA obligations. You may request deletion at any time. Data Security
We maintain reasonable administrative, technical, and physical safeguards designed to protect your information. However, no method of transmission or storage is completely secure, and we cannot guarantee absolute security. Any transmission of information is at your own risk.

Children’s Privacy
Our Website, products, and services are intended for individuals 18 years of age or older. We do not knowingly collect personal information from anyone under 18. If you believe we have collected information from a minor, please contact us so we can delete it.
